Our first project on my first team on my first company just went live. There WAS a test server, and the features have been tested, but somehow almost every test failed to point that the features were either broken, or not meeting the specs. That, and the specs changing during and after it going into production, among other things.

Well, not everyone, but still... It's been like that for a year or so. I wonder how chaos hasn't taken control yet. (to be honest, I have no idea what I'm doing in this group and haven't left just because I feel tempted to do something...)
